Major Duties include but are not limited to the following:
Evaluates and monitors medications for appropriateness of therapy and patient's response to therapy based on relevant therapeutic endpoints by means of patient interviews, physical and functional assessment, and diagnostic tests. Prescribes and modifies medication regimens in a safe, effective, and economical manner. Provides these collaborative drug therapy management services in accordance with the individual's approved scope of practice and prescriptive authority. Evaluates drug therapy through direct patient involvement, with clinical assessment and objective findings relating to patient's responses to drug therapy and communicating and documenting those findings, interventions and recommendations to appropriate providers and in appropriate records (e.g. patient's medical record). Prescribes, continues, modifies, or discontinues drug therapy for both inpatients and outpatients in accordance with the individual's approved scope of practice and prescriptive authority. Has knowledge and skills necessary to provide care appropriate to the age of the patient served. This includes knowledge of the changes associated with aging and the ability to incorporate these factors in the treatment needed by different age groups. Sensitivity to the special needs of all patients with respect to age, developmental requirements, and culturally related factors must be consistently achieved. Provides education to physicians, nurses, and other health care professionals. Functions as a therapeutic consultant to providers and health care teams to develop, implement, and maintain effective mechanisms to promote the safe, economical and rational use of medications. Assists in the management of medical emergencies, adverse drug reactions, and acute and chronic medication management. Maintains professional competency on drug therapies and innovations in pharmacy practice pertinent to the use of anticoagulants. Provides medication counseling to patients and/or caregivers. Evaluates the abilities of adult and geriatric patients to understand medication instructions (i.e. ability to learn, sensory limitations, past knowledge of condition, and obstacles to following treatment regimen) and provides oral and written counseling on their drug therapy. Assists in the development, coordination and provision of drug therapy in-service programs, staff development programs, patient education programs, and other system-wide drug information programs. Serves as preceptor for pharmacy students and pharmacy residents as assigned. Serves as a leader and subject matter expert in the oversight, design, implementation, and function of the anticoagulation management program. Participates in quality assurance and medication use evaluation programs particularly as they pertain to anticoagulation therapy management and reports to relevant Health-System committees (e.g. Pharmacy and Therapeutics committee). Manages the centralized Anticoagulation Clinic at VA Eastern Kansas Healthcare System (Leavenworth and Topeka divisions). Develops medical center policy, practices, protocols, and procedures related to the use of anticoagulants in all settings of care using an interdisciplinary approach in order to maintain compliance with all applicable standards of care set forth by applicable VHA Handbooks and Directives and applicable accrediting bodies (e.g. TJC, OIG) and ensures they are current and reflective of current evidence-based practice. Promotes learning and unified practice for the anticoagulation management program through educational initiatives, regular staff meetings, and journal clubs. Performs proper recordkeeping and maintains proper documentation when required or appropriate for all clinical activity and patient interventions. Ensures sensitive data for patient files, both printed and electronic, remain confidential. Ensures ADP systems for pharmacy practice and patient care are utilized for official duties and remain secure. Actively participates in various quality management and performance improvement activities such as drug utilization evaluations, drug class reviews, adverse drug reaction monitoring, new drug request reviews, prior approval/non-formulary drug reviews, ongoing drug monitors, and a variety of other activities associated with the provision of pharmaceutical care. Designs and/or participates in research studies under FDA guidelines that have undergone review and approval by the Research & Development Committee and the Investigational Review Board. Serves as a member of various pharmacy and health care system clinical care committees and workgroups. Work Schedule:

Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. Experience:
Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following:
supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Must have 1 year of experience equivalent to the next lower grade level. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
