Social Worker (Caregiver Support Coordinator)

This Caregiver Support Program Coordinator is assigned to the Topeka VA Medical Center. Duties include but not limited to:
Primary responsibilities are to provide clinical evidence-based services and interventions; program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality-improvement activities and evaluation/consultation. Will function in support of caregivers, Veterans and family members throughout the Medical Center and in the community and will support the development and implementation of General Caregiver services. This includes development of a strategic plan for expansion of the General Caregiver Support Program and conducting frequent needs assessments to implement education, wellness, and support groups. In addition, the Coordinator must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations. The Coordinator, in conjunction with the Program Manager, is administratively responsible for the clinical programming and daily operations of the General Caregiver Support Program. Develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. May be responsible for the program's budget and various control points assigned to the program within the assigned areas. The Coordinator manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. Plans, organizes and directs administrative, operational, caregiver, Veteran and personnel activities for the program. Experience that demonstrates the advanced practice skill level in social work. At least two years of experience should be in an area of specialized social work practice. The Coordinator must be independently licensed by a state to practice at the advanced practice level. The Coordinator will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. Practicing at the advanced level and has specialized knowledge of practice that can be differentiated from the independent practice social worker by the ability to expand clinical knowledge in the profession, administer a facility General Caregiver Support Program, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. Requires advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. As an advanced practice level clinical social worker, the Coordinator is assigned to work independently with multiple VA healthcare services and with community agencies, leaders and organizations where he/she has no access to direct social work clinical supervision. Responsible for developing and establishing linkages and collaborates on VA caregiver support efforts with community healthcare and social service agencies. The Coordinator is administratively responsible and accountable for the clinical and program development and effectiveness of the facility General Caregiver Support Program. Other duties as assigned. Successful completion of master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. (Transcripts must be provided). Licensure:

GS-12 Experience/Education:
To qualify, candidates must possess experience that demonstrate possession of advanced practice skills and judgment. At least two (2) years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. GS-12 Licensure/Certification:
In order to qualify for this level, you must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and you must be able to provide supervision for licensure. In addition to the experience above, candidates must demonstrate all of the following KSAs:
1. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. 2. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. 3.Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. 4. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. 5.Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
